Our mission is to improve the health of children, families and seniors by providing access to care through health insurance enrollment, patient education and advocacy. 

Everyone deserves access to quality healthcare—regardless of their income, language, age, or circumstances. Yet for thousands of Solano County residents, navigating the healthcare system can be overwhelming and filled with barriers.

That is why Community Health Initiative removes obstacles to care for nearly 9,000 of Solano County’s most vulnerable children, families, and older adults each year.

Our bilingual team of Medi-Cal, Covered California, and Medicare experts helps low- and moderate-income residents obtain and maintain health coverage by guiding them through complex enrollment and renewal processes, submitting required documentation, and resolving issues that could otherwise result in delayed care or loss of coverage.

But our work extends far beyond insurance enrollment. We help individuals and families establish ongoing healthcare by connecting them with primary care providers, ensuring access to in-network services, and linking clients to critical behavioral health resources. When language barriers, technology challenges, or complicated healthcare systems stand in the way, we step in to schedule appointments, coordinate transportation, and advocate on behalf of our clients so they can receive the care they need.

We also empower clients with the knowledge and skills to navigate healthcare independently, providing education on health plan benefits, online portals, and available community resources.
